To register for our online services you will need to complete an ‘Access to Online Services’ form. You can collect a form from one of the surgeries or you can click the following link and print the Access to Online Service Form. This form is not currently available to complete online.

Please read and retain pages 1-4 for your information.

Complete pages 5 & 6 and bring them along with page 7 and relevant ID documents to one of the surgeries. Information on the ID documents can be found on page 6.

Access takes between 2 and 20 working days to process, depending on the level of access required – see page 6.

Once you are registered you will be able to:

• Order your repeat prescriptions
• Cancel an appointment
• Change your contact details
• Review your medications and known allergies
• View your medical records (if requested)
