Routine Appointment Forms

From 1st October, NHS England requires all GP practices to be fully accessible to patients during core hours (Monday to Friday, 8am–6:30pm).

Pennine Medical Centre is ahead of the curve — our new triage form is already live, making it easier than ever for patients to book routine appointments within our core hours.

Named GP

All of our patients are allocated a named GP. This does not affect your wishes to see a preferred GP.

For patients aged 75 and over, the named, accountable GP is responsible for:

  • working with health and social care professionals to deliver a care package that meets the needs of the patient
  • ensuring that these patients have access to a health check

Patients can still choose to see any GP in the surgery. We will make reasonable efforts to accommodate their preference.
