The practice will accept urine samples from patients who have been asked to bring one in by a clinician.
Patient must complete a short questionnaire, which is available from reception.
Patient must label their own sample pot.
Any samples handed in unlabeled and without all the information required on the questionnaire will be turned away or destroyed.
Clinicians are unable to make a decision on possible treatment without all the required information as a urine sample alone does not give the full picture.
