Routine Appointment Forms

From 1st October, NHS England requires all GP practices to be fully accessible to patients during core hours (Monday to Friday, 8am–6:30pm).

Pennine Medical Centre is ahead of the curve — our new triage form is already live, making it easier than ever for patients to book routine appointments within our core hours.

Request a routine appointment

Use this service to get routine help from a doctor, nurse or other healthcare professional.

Your request will be triaged by a GP based on your answers to all the questions. You will be contacted by a member of the team via email, text or phone call with an appointment date and time. Please note this will not be a same day appointment.

We aim to respond within 48 hours during our opening times.

You can use this service if you:

  • are registered at the surgery

Due to the changes around access to GP surgeries, we anticipate a rise in demand for routine care, which in turn could place greater pressure on our services, increasing routine appointment wait times.

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
  • the type of routine appointment you need, such as a blood test (if known)

Call 999 or go to A&E now for any of these:

Signs of a heart attack
chest pain, pressure, heaviness, tightness or squeezing across the chest

Signs of a stroke 
face dropping on one side, can’t hold both arms up, difficulty speaking

Sudden confusion (delirium) 
cannot be sure of own name or age

Suicide attempt 
by taking something or self-harming

Severe difficulty breathing 
not being able to get words out, choking or gasping

Heavy bleeding 
spraying, pouring or enough to make a puddle

Severe injuries 
after a serious accident

Seizure (fit) 
shaking or jerking because of a fit, or unconscious (can’t be woken up)

Sudden, rapid swelling 
of the lips, mouth, throat or tongue

British Sign Language (BSL) speakers can make a video call to 999.

Deaf people can use 18000 to contact 999 using text relay.
