Registering with the surgery
Registration pack
The following documents contain all the information you should need to register with us. Please read all the information carefully and then:
- print and complete the new patient questionnaire (adult / child)
- if you are on repeat medications, please attach a repeat prescription slip to your questionnaire
- print and complete the GMS1 registration form
- attend the Mossley or Greenfield surgery
- use the health monitor in the reception area
- hand into reception your completed questionnaire, GMS1 form and one of the slips from the health monitor

Temporary registration
If you are ill while away from home or if you are not registered with a doctor, but need to see one, you can receive treatment from a local GP practice as a temporary patient.
To register as a temporary patient simply contact the local practice that you wish to use. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
