There are a few ways to manage your health online – see below
Via our website using an online form
- request an appointment
- request a sick note
- ask about test results
You do not need an account to use this website.
Via online services, eg NHS App or Patient Access
To gain access to online services you will need to complete an application form, which is available from the practice or can be download here:
- Access to online services (including proxy access) – application form – patients aged 16+
- Access to online services (including proxy access) – application form – patients ages 11-15
The application form contains all the information you should need to understand what an online patient account is and how to access it once access is granted. We ask you that read the information carefully and retain the first three pages of the application form so that you can refer to it later.
Once the application form has been processed and you have been granted online access, you will use one of the online access service providers to view your account. Details about the various online access service providers will be sent with your access codes once access has been granted. A list of all the available online service providers can be found here: https://www.nhs.uk/nhs-services/gps/online-health-and-prescription-services/
Our website is not an online access service provider and you cannot view your online services account via our website.
Services available using online access are:
- order repeat prescriptions
- view your test results
- access your health record
- check or cancel appointments
- see your vaccination record
